Custome Query Report base on time spent

How can I run a custom query base on RequestID , Description(the text box in the add time entry window) , Technician , CompletedDate?
or is it possible to run a normal report base on fields mentioned abow?
for you information the description (multi line) text field is different as same one in new request window.
