Custom Triggers - Email Notification

I am trying to setup a custom trigger for when a certain template is used.

I have configured the trigger, provided a name and description and set the Execute on actions to Created.
Execute during: Any Time
Cascade Execution: execute next rule

The criteria for the trigger is as follows:

Execute rule when the criteria is met: Every Time
Template - IS - Printer Toner Request

Actions are: 
Notification Action with custom template.



My issue is, that when the tickets are logged the trigger is not triggering when this template is used.  How can i ensure that this happens?
