Is it possible to create a custom script where on form submission it fires off a task template we have saved?  Additionally, is it possible to grab information from fields on the form to enter data into certain fields on the task?

For example, we have a task template called "Locator Needed" and we have a field on the form for new user creation called "Locator Needed?", which is a yes/no drop-down.  We would like to see if it can add the task if "Yes" is selected for this field on the form.

We also have a field on the form called "Start Date" that's filled out by the user and would like to have the date entered in this field placed in the "Scheduled Start" field on the task.  This would go into the Scheduled Start "From" field, the To field would remain blank.

We also have a "Legal Full Name" field on the form we would like placed in the comments on this task.

Is this possible with SDP?  If it's, how would you script this?
