Custom Form Fields Specific To Request Template

AdventNet is on the right track with request templates, but could be benefited from adding the ability to display certain form fields for input based on the type of request. The default, built-in, "New Joinee" request template is a good example of this.

When you select the "New Joinee" template, the "DESCRIPTION" of the request is populated with specific text to this type of request. Instead of custom text in the DESCRIPTION along the lines of:

Employee Name :
Employee Title :
Date of Birth :
E-nail id:
Department :
Location :

Each of these fields of text should be able to be defined as "Request - Additional Fields". Only for each additional field, the admin must select which TEMPLATES this additional field should be shown with, to include an option for "All Templates" separate from "All Requests" (not all requests use a template).

"Date of Birth" would be a "Request- Additional Field" that is ONLY shown on the "New Joinee" template, not the "Firewall Change", "Spam Email", or "Virus Detected" templates. By creating additional request fields that are selectively shown based on the type of request, the field inputs become more reliable, with the option of adding drop down boxes, etc. You would need to increase your maximum number of "Additional Fields" from the current 12, as if you had several templates, you could quickly come up with 2-4 fields PER TEMPLATE that are unique. The free form text in the DESCRIPTION is a good start, but Request-Specific form fields is where the real benefit is to be had.
