Controlling the order of 'additional fields' on the Request

First time poster !
I'm tasked with creating a new layout of fields so that our request form looks it best. That means I need to move the fields around so they are grouped in a logical order. We are only talking about the 'Category' section of the form.
Now, I have given each of the 12 "additional fields" a label the corresponds to the number next to each label when in the 'Requst - additional fields' view. 1 =1 and so on.
When I do a 'request form preview' the fields are arranged like this.
Category
Sub Category
Item
1 10
11 12
2 3
4 5
6 7
8 9
It's not sorted in any order i can recognize. I need to know how I can make the form show the fields in the following order:
1 2
3 4
5 6
7 8
and so on.
I'm not going to use all the fields available, and when leaving some fields empty it becomes even more confusing and impossible to work with.
I hope this is enough to describe the issue I have. Else I can attach screenshots and what else. Just let me know. Please.
