We do reglarly scheduled maintenence for many clients and much of this involves working through a checklist like:

I would like to use the scheduled requests along with a checklist incorporated in the request. Unfortunately check box type fields do not appear to be an option. I saw that I could include HTML in a cutsomized request description but this would involve a bit of code to record the check/uncheck state of each item and I have no real clue where to start with this.

Could we get check box type fields as a type of field in the Request - Additional Fields to add to Request Templates? In my case I don't need any logic or rules to work with the check boxes, they just need to be recorded.  

 

Radio buttons may be be nice too in some circumstances too and may often be a more efficient way of presenting a small list especially if there is some flexibility in laying out the radio button list horizontally or vertically.
