Can't add a notification profile to a device.

Can't add a notification profile to a device.

I'm not able to assign a notification profile to a device, everytime I try it and hit the next button I get an page saying "error!".
The profile does not include any blanks or special characters.
Any idea how I can fix this ? Please Help
