I have created incident template from Admin - Incident Template - New Template. Attached snap shot.

I have created business rule from Admin - Organizational Details - Business rule. Attached snap shot.

Issue,

I have created incident from user end with template that i have created 

Result 

Business rule is not working

However business is rule working is i will create incident with default template.

I have attached snap shots as well.
