Auto Disenroll

We have several users that have enrolled, but when they go to unlock their account or change their password, they get a User Must Enroll message. We have verified cases of this from our helpdesk. Has anyone run into this? Is it possible that it's a license issue where we've exceeded out license? Is it possible that the system is disenrolling users on it's own? Is it possible to get a disenrolled users report?
