Additional Fields Not Visible - 7.5 Beta Version

Any "New Request Additional Filed" is added in beta version, the same wont appear in New Request Form. Could you please let me know the fix for the same.
Also i don't see Request Form Customizer and Request Preview. Has this been removed in this version.
