Adding User defined fields into new Incident Template (Requester Details)

Hello,
We would like to display a user defined field that we have setup pulling from AD called (Employee ID) in tickets under the Requester Details area. As shown Below in Figure 1

We are able to see the information if we click on the edit User details button and then search fro the user.
As shown  in Figure 2 and 3

But as stated above we would like this to show in the ticket just as Contact number and Department do.
