Is is possible to add the option for 'Last 12 Months' to Matrix and other reports?

I have a need to build a report which is automatically generated which would show Request Count by Level for each month.  The best I can currently generate is 'Last Quarter'.

Further, is it possible to show a Graph to illustrate this data in the same report?
